Emsculpt NEO for chronic lower back pain and core weakness.

A large body of pain medicine research identifies weakened core musculature as a primary driver of chronic lower back pain. Emsculpt NEO builds core strength at a level voluntary exercise alone often can’t reach — and many of our patients see meaningful pain reduction within weeks.

How the treatment works

The mechanism — plain language.

The deep core muscles — transverse abdominis, multifidus, internal obliques — function as a natural support corset for the lumbar spine. When these are weak (from prolonged sitting, post-injury deconditioning, pregnancy, or age), the spinal load shifts to the joints and discs, causing chronic pain.

Traditional core exercises often miss the deepest stabilizing muscles. Planks, crunches, and dead bugs all help but require months of consistent work to fully re-engage the deep core.

Emsculpt NEO uses HIFEM technology to force contraction of the entire core musculature — including the deep stabilizers — at 20,000 contractions per 30-minute session. This is a level of muscle engagement no voluntary exercise can match.

Most patients with chronic lower back pain see improvement within 2–3 sessions. By the end of a 4-session protocol, many report significantly reduced daily pain and improved functional movement.

Clinical evidence

What the research shows.

Published clinical research has documented Emsculpt’s effectiveness for chronic non-specific low back pain via core muscle strengthening. In one published case series, the majority of chronic low back pain patients reported reduced pain scores and improved functional outcomes after a 4-session Emsculpt protocol.

Is this right for you?

Who qualifies.

  • Chronic lower back pain (typically defined as 3+ months of recurrent pain)
  • Pain related to core weakness or postural stress (not from acute injury, disc herniation, or systemic conditions)
  • Failed conservative treatment with stretching, physical therapy, or oral pain management
  • Otherwise healthy adults seeking a non-surgical, non-pharmaceutical option

Not the right approach if:

Your back pain has a specific identifiable cause (disc herniation, sciatica, scoliosis, vertebral fracture) — those require different medical evaluation. You have active spinal inflammation. You have electronic implants in the area. You can’t tolerate intense muscle contractions due to other medical conditions.

Common questions

What patients ask before starting.

Is this an alternative to physical therapy?
It complements physical therapy beautifully. Many patients use Emsculpt to accelerate core engagement that traditional PT exercises target. Most PT practices welcome Emsculpt as an adjunct treatment.
Will the relief last after I stop treatment?
If you maintain the core strengthening through ongoing exercise, yes. If you stop all core engagement, the muscles will gradually weaken again — same as any conditioning. Most of our chronic back pain patients continue with 1 session every 3 months for ongoing maintenance.
How soon will I feel relief?
Many patients report meaningful pain reduction by session 3. Full protocol results are typically clear by 8 weeks after the final session as the deep muscles continue strengthening.
What if I have a herniated disc?
Emsculpt is not appropriate as primary treatment for disc-related pain — that requires neurosurgical evaluation. However, for patients with stabilized disc issues whose pain is now predominantly from core weakness, Emsculpt can help. We assess at consult and may refer for medical evaluation first.
Is it covered by insurance?
Currently no — Emsculpt for back pain is not covered. Self-pay. We provide written pricing at consult.
Does Dr. Lam supervise this?
Yes. Medical applications of Emsculpt are reviewed and approved by Dr. Lam before treatment.
